Zyloric-300 contains Allopurinol 300mg as a higher-strength tablet formulation of the same xanthine oxidase inhibitor available in the standard 100mg strength, offering a convenient single-tablet maintenance dose for patients with moderate-to-severe hyperuricemia, tophaceous gout, or gout associated with high uric acid turnover conditions, eliminating the need for multiple lower-strength tablets and significantly improving patient compliance and adherence to long-term urate-lowering therapy. At this therapeutic dose of 300mg once daily, Allopurinol and its active metabolite Oxypurinol provide sustained and comprehensive inhibition of xanthine oxidase throughout the dosing interval, effectively reducing serum uric acid levels to the target of below 6mg/dL (360 µmol/L) recommended for most gout patients, or below 5mg/dL in patients with severe tophaceous gout, thereby preventing acute gouty attacks, reducing tophus size, and protecting against urate nephropathy and urolithiasis with long-term use. It is indicated for moderate-to-severe chronic tophaceous gout, recurrent gouty arthritis requiring higher urate-lowering efficacy, hyperuricemia associated with hematological malignancies and chemotherapy-induced tumor lysis syndrome prophylaxis, recurrent uric acid nephrolithiasis, and conditions of markedly elevated uric acid production where the 100mg dose alone is insufficient to achieve target serum urate levels despite adequate titration. As with all Allopurinol formulations, Zyloric-300 should never be commenced during an acute gout attack, mandatory colchicine or NSAID co-prescription for the initial 3–6 months is essential to prevent attack precipitation during urate mobilization, dose reduction is critical in renal impairment, adequate hydration should be maintained, and patients should be counseled about the rare but serious risk of allopurinol hypersensitivity syndrome requiring immediate discontinuation upon appearance of any skin rash or systemic hypersensitivity reaction.
