YAZ Tablets contain a fixed-dose combination of Ethinylestradiol 0.02mg (a synthetic estrogen) and Drospirenone 3mg (a fourth-generation progestogen with unique antimineralocorticoid and antiandrogenic properties), representing a low-dose combined oral contraceptive (COC) that works through three complementary mechanisms — primarily inhibiting ovulation by suppressing the hypothalamic-pituitary-ovarian axis and preventing the LH surge, secondarily thickening cervical mucus to impair sperm penetration, and additionally altering the endometrial lining to prevent implantation, providing highly effective contraception with a Pearl Index of less than 1 when taken correctly. Drospirenone’s unique antimineralocorticoid activity (comparable to 25mg of spironolactone) counteracts the estrogen-induced sodium and water retention, reducing bloating, weight gain, and fluid-related side effects commonly associated with other COCs, while its antiandrogenic properties make YAZ particularly beneficial in women with androgen-dependent conditions such as acne, seborrhea, and hirsutism, distinguishing it from most other combined oral contraceptives. Beyond contraception, YAZ holds additional approved indications for the treatment of moderate acne vulgaris in women seeking oral contraception, and Premenstrual Dysphoric Disorder (PMDD), making it one of the few oral contraceptives with multiple officially approved non-contraceptive therapeutic indications. Administered as one tablet daily for 28 consecutive days (24 active pink tablets followed by 4 placebo white tablets) without a pill-free interval, YAZ is contraindicated in women with thromboembolic disorders, hypertension, migraine with aura, hepatic dysfunction, renal or adrenal insufficiency (due to risk of hyperkalemia from drospirenone’s antimineralocorticoid effect), pregnancy, and undiagnosed vaginal bleeding, with nausea, breast tenderness, spotting, and mood changes being among the most commonly reported side effects.
