Description
Xarelto contains Rivaroxaban, a novel oral anticoagulant (NOAC) belonging to the class of direct Factor Xa inhibitors, which works by selectively and reversibly blocking Factor Xa — the pivotal enzyme at the convergence of both the intrinsic and extrinsic coagulation pathways — thereby interrupting the coagulation cascade, preventing thrombin generation, and inhibiting clot formation without requiring routine coagulation monitoring or dose adjustments based on INR values, unlike traditional anticoagulants such as warfarin. It is indicated for a broad range of thromboembolic conditions including prevention and treatment of deep vein thrombosis (DVT) and pulmonary embolism (PE), stroke prevention in non-valvular atrial fibrillation, prevention of venous thromboembolism (VTE) following hip or knee replacement surgery, and reduction of cardiovascular events in patients with coronary artery disease or peripheral artery disease when combined with aspirin. Available in strengths of 2.5mg, 10mg, 15mg, and 20mg, dosing varies by indication — for example, 20mg once daily with the evening meal for atrial fibrillation, or 15mg twice daily for the first 3 weeks followed by 20mg once daily for DVT/PE treatment — offering a convenient fixed-dose oral regimen without the dietary restrictions associated with Vitamin K antagonists. Xarelto should be used with caution in patients with renal impairment, hepatic disease, and significant bleeding risk, is contraindicated in pregnancy, and lacks a specific reversal agent in most settings, though Andexanet Alfa is available for life-threatening bleeds, with major bleeding being the most serious adverse effect requiring vigilant clinical monitoring.
