Description
Vastarel MR contains Trimetazidine Dihydrochloride 35mg as a modified-release tablet, representing a unique anti-ischemic and cytoprotective agent that works at the cellular level by inhibiting the enzyme 3-ketoacyl-CoA thiolase (3-KAT), thereby shifting cardiac energy metabolism from fatty acid oxidation to the more oxygen-efficient glucose oxidation, resulting in improved myocardial energy utilization under ischemic conditions without affecting heart rate or blood pressure. Unlike conventional anti-anginal drugs, Trimetazidine does not act hemodynamically but instead protects the myocardial cells from the damaging effects of ischemia-reperfusion injury by maintaining intracellular homeostasis, reducing intracellular acidosis, and preventing the calcium overload and free radical damage that occurs during ischemic episodes. It is primarily indicated as add-on therapy for stable angina pectoris in patients inadequately controlled with first-line anti-anginal agents such as beta-blockers or calcium channel blockers, and also finds use in tinnitus, vertigo of vascular origin, and chorioretinal vascular disorders. Administered orally as one tablet twice daily with meals, Vastarel MR is generally well tolerated; however, it is contraindicated in patients with Parkinson’s disease, parkinsonian symptoms, tremors, restless leg syndrome, and severe renal impairment, as Trimetazidine has been associated with movement disorders with prolonged use.
