Description

Uperio contains a fixed-dose combination of Sacubitril (a neprilysin inhibitor) and Valsartan (an angiotensin II receptor blocker), representing a novel drug class known as Angiotensin Receptor-Neprilysin Inhibitor (ARNI), specifically indicated for the treatment of chronic heart failure with reduced ejection fraction (HFrEF) in adults to reduce the risk of cardiovascular death and hospitalization. Sacubitril works by inhibiting the enzyme neprilysin, which is responsible for breaking down natriuretic peptides, thereby increasing their levels and promoting vasodilation, natriuresis, diuresis, and reduced cardiac fibrosis; while Valsartan simultaneously blocks the renin-angiotensin-aldosterone system (RAAS) by antagonizing AT₁ receptors, reducing vasoconstriction, aldosterone secretion, and cardiac remodeling. Available in strengths of 24/26mg, 49/51mg, and 97/103mg (equivalent to 50mg, 100mg, and 200mg respectively), it is administered twice daily with the dose titrated every 2–4 weeks to the target maintenance dose as tolerated by the patient. Uperio is strictly contraindicated in combination with ACE inhibitors (a 36-hour washout period is mandatory), in patients with a history of angioedema, bilateral renal artery stenosis, and is absolutely contraindicated in pregnancy due to fetotoxicity, with hypotension, hyperkalemia, and renal impairment being the most commonly monitored adverse effects.
